Creating a record for a patient guardian, proxy, or caregiver enables you to send eConsent forms for electronic signature in conjunction with or on behalf of a study participant. See Start the Consent Process for more information on consenting participants. You can also share documents with guardians, proxies, and caregivers. See the Sharing Documents With Study Participants help topic for more information.
- Navigate to Administration > Patients and open the profile for the patient.
- Expand the Guardians, Proxies, and Caregivers section and select Create.
- Select the Signatory field and complete one of the following steps:
- To add an existing signatory, select from the list.
- To create a new signatory, select + Create Signatory and complete the required and any optional fields.
- Select Save.
- Complete the Role field.
- Select Save.
Signatories can also be created by navigating to Studies > Select Study > Participants, then select Create Signatory from the Participant’s Action menu.
Signatory Roles
Each patient has different support needs; select the appropriate signatory for your participant’s specific situation.

| Guardian / LAR | Individual legally authorized to consent on behalf of the participant (ex., minor, incapacitated adult). Guardian/LAR signatories can sign alongside or on behalf of the Participant depending on how the document is configured. | Multiple guardian signatory records can exist for a participant, but note that a maximum of two Guardian signatures can be added to eConsents where multiple guardian signatories are needed. |
| Proxy | A person legally designated (ex., via Power of Attorney) to make medical decisions, including research participation, for an incapacitated adult. SiteVault prevents sending consent forms to a Proxy and Participant at the same time. A proxy signs on behalf of the Participant, never in addition to the Participant. | N/A |
