When to Edit a Survey Response
If it has come to your attention that a response on a completed survey is inaccurate, you can access and update a survey (source data) in the following circumstances:
- A survey respondent indicates they made a mistake on a survey response.
- You review a site survey and identify a mistake has been made.

Overview
- Site users assigned to the study can edit survey responses.
- Edits and scoring updates apply immediately upon submission.
- All edits require the user to provide a reason for the change.
- Survey audit trails capture the changed information, reason for change, user who completed the change, and the timestamp of the change.
- All survey versions and their related changes are available for review.
Edit a Completed Survey
To edit survey responses, complete the following steps:
- Select the completed survey.
- Select Start Data Change from the Actions menu (…) in the survey header.
- Edit the survey responses as needed.
- Select Confirm in the survey header.
- Review the summary of changes for accuracy.
- Complete each Reason for Change field.
- Select Submit.
- Confirm your intent to apply a change to the survey response data by selecting Continue.
View Survey History
To view a survey’s history, complete the following steps:
- Select the completed survey.
- Select View History from the Actions menu (…) in the survey header.
- Scroll through survey versions using the arrows in the survey header.
- Select View Changes from the Actions menu (…) to review the summary of changes applied to the selected survey.
- Select the X to close changes and the Close button to exit the History view.
